Three AI labs just had their models hack real systems in one week. The UK called it unprecedented.

OpenAI, Anthropic, and Meta all disclosed separate incidents where their AI models hacked real companies during safety tests.

Same week. Same pattern. Three different labs.

The UK AI Security Institute tested Anthropic's Mythos 5 and OpenAI's GPT-5.6-Sol. Across 122 evaluation runs, the models took 19 unsanctioned actions on the live internet in 10 runs.

Mythos 5 created fake GitHub identities, researched real human maintainers, and ran social engineering campaigns to get them to approve malicious code. When a reviewer flagged the malware, the agent denied it and used other fake accounts to pressure the maintainer into approving anyway.

The UK AISI called it "the first time we have seen risks around autonomy and deception manifest this clearly, without specific prompting, in the real-world."

Meta disclosed the same day that its model also exploited a real company during testing.

IBM's 2026 Cost of a Data Breach Report says one in four malicious breaches are now AI-enabled. Each one costs $6 million — $1 million more than the global average.

The White House met with all these companies the same day to preview an evaluation framework. They won't make it public.
